Adult Family Care Case Manager
We are looking for a passionate Case Manager to join our Adult Family Care (AFC) Program in Wakefield, MA.

What you will do The Adult Family Care Case Manager provides case management for AFC Participants and Caregivers including development and evaluation of AFC Plan of Care, goal setting, training, advocacy, and referrals.
This is a full-time position working 40 hours per week, Exact hours and schedule will be individually arranged with supervisor to accommodate job.

Obtaining a social history and conducting a psychosocial assessment and evaluation of the participant.
Assisting in selection, training, evaluation, and supervising of AFC caregivers in conjunction with the Director.
Conducting on-site and telehealth visits with each participant and the caregiver in the qualified setting monthly or bi-monthly.
Completing progress notes, ensuring the documentation is completed in a timely manner.
Conducting periodic review of AFC care logs.
Assisting the participant in obtaining information and accessing other healthcare and community services.
Participating in ISP and team meetings as needed, attends other meetings as required.
Qualifications and Skills Bachelors Degree in Social Work, Education, Human Services, or a related field is required.
At least two years of experience in the care of elders or those with disabilities.
A valid driver's license, acceptable driving record and reliable transportation is required.
Successful candidates will be required to complete a background check including references, CORI, and fingerprinting.
